4) Debarred by PE during last 5 years
5) Termination of incomplete contract for poor performance by PE during last 5 years
6) Extension of Time beyond original contract period by PE during last 5 years
7) Imposition of Liquidity Damages (LD) by PE during last 5 years
8) Extension of contracted Defects Liability Period (DLP) by PE during last 5 years
9) Declared bankrupt during last 5 years.
