a) Concerned Trade License (Updated).
b) VAT Registration & Income Taxes Clearance eTIN Certificate(Updated).
c) The tenderer should be a Manufacturer / Importer / Supplier / Distributor.
d) The minimum amount of liquid assets (17-5-22 to 30-5-22) Tk. 30 Lakhs as Bank Solvency & Bank statement (Original Copy), fiscal year 2020-21.
e) Experience certificate minimum 02 years of similar supply.
f) The satisfactory completion of Supply ok similar Goods of minimum Tk. 30 Lakh of 500 bed Government / Semi-Government/Autonomous hospital under minimum 01 contract within the last 3 (three) years. f) The satisfactory completion of Supply ok similar Goods of minimum Tk. 10 Lakh of 500 bed Government / Semi-Government/Autonomous hospital under minimum 01 contract within the last 3 (three) years.
g) The bidder must provide manufacturer's authorization letter or authorization from the sole distributor (Distributor must be authorized by the manufacture to issue such authorization letter) of the particular item (which-ever applicable).
h) The tenderer must mention manufacturer's name, country of origin, model number must be printed on the Instrument & Equipment's in embrossed / engraved. Temporary sticker will not be accepted.
i) The tenderer must submit original catalogue / brochures in duplicate.
j) Membership / chamber of commerce certificate.
k) The availability of after sales service, spare parts for the equipment supplied.
In case of delay of supply of goods LD will be applicable as per PPR 2008.
