A. The Tenderer shall submit the following additional documents:

General experience of the supply of Goods as Supplier shall be last 03 years.

Specific experience with Work order & Agreement of at least 02(two) contract(s) successfully completed within the last 05(five) years, Total value of at least Tk. 382.00 Lac.

Liquid assets i.e credit line(s) (Form PG3-8) of the Tenderer shall be Tk 382.00 Lac.

Up-to-date 2024-25 Trade License, TIN, e-BIN Certificate; & NID.

Bread manufacturer Bakery & BSTI License.

Manufacturer’s  License and premises license is required only for the item Bread.

Last 3 years Litigation History.

Last 3 years Audit Report.

Brochures.

Manufacturer Authorization Letter or Contract deed.

Written Signature Authorization.
B. Special Instruction:


Goods should be supplied as per samples kept in RMCH Kitchen Store.



The expiry date shall not be more than 03(three) days from the date of production.



Date of manufacture and expiry date should be printed.



The polythene wrap should be printed with the name of RMCH Institute.



Supply of good quality & fresh Bread should be ensured.



Bread must be Approved/Accepted by the committee.
